Quality of care for public and private hospitals in Korea

Abstract
Purpose: This study aimed to compare the accessibility and quality of inpatient care in public and private hospitals in Korea.

Methods: The 2015 statistics of public medical institutions published by the Ministry of Health and Welfare and the National Medical Center and quality assessment data (2015 or nearest year) of the Health Insurance Review and Assessment Service was used. The general characteristics, accessibility for inpatient care (number and proportion of hospitals with assessment grade for each assessment item), and quality of care (number and proportion of hospitals with standard grade) between public and private hospitals were compared.

Results: Public hospitals tend to be located in non-metropolitan area and provide more medical inpatient care for vulnerable patients (Medicaid patients and elderly).
Total proportions of hospitals with assessment grade for all assessment items except coronary artery bypass graft were higher in public hospitals. However, proportions of hospitals with assessment grade were commonly higher in private hospitals for tertiary hospitals and general hospitals with more than 500 beds. In addition, numbers of small public hospitals with assessment grade were less.
Total proportions of hospitals with standard levels of quality for acute myocardial infarction, prophylactic antibiotics for surgery, intensive care unit, pneumonia, colorectal cancer, breast cancer, surgical volume, and long-term care were higher in public hospitals. However, there were conflicting results regarding the hospital types.

Conclusions: Many public hospitals have achieved the accessibility for inpatient care and standard levels of quality in some acute diseases, surgeries, and long-term care. However, efforts to improve accessibility for inpatient care in small public hospitals (general hospitals under 500 beds and hospitals) and quality improvement in large public hospitals are needed.
